Summary:
Weekday and/or weekend and/or PH
8 am to 8 pm
The working hours may vary depending on the roster.

Job Scope
• Attend and provide updates at the Multi-Disciplinary Team meetings;
• Receive nursing handover from inpatient nursing team;
• Conduct virtual onboarding for example: phone call with patient and handle enquiries relating to treatment/ medications/ infection control and ensure patients settle at home safely;
• Ensure proper documentation of patients in system;
• Conduct daily regular call in to patients to monitor patients;
• Perform admission, discharge, and transfer on system when needed;
• Monitor the vital signs of patients using tele-monitoring system;
• Liaise with external service provider;
• Facilitate ad-hoc home care;
• Support other admin/ops related activities (when required)
• Undertake duties which may be assigned by the staff member depending on the requirements

Job Requirements:
-Possesses valid SNB, BCLS
