Care Coordinator Associate (Queenstown/5 days/Up to $2.6k/Diploma)

• Diploma holder with 1 year of relevant experience welcome
• Salary range $1800 - $2600
• 5 days work week

• Planning and providing various activities in community health posts.
• Providing health education, counselling to patient on preventive lifestyle modification education.
• Motivating patients to take up preventive health interventions and seek appropriate care.
• Working with Care Managers to assess patient’s needs and their current support system.
• Understand the services available and be able to explain and encourage patients for enrolment.
• Conduct follow-up review/ home visits to ensure smooth coping.
• Maintain team’s database and ensure timely updates for recruited patients.

• Candidates must possess at least a Nitec/ Diploma in Nursing, Social Work, Health Services Management or relevant.
• Must have at least 1 to 3 years of working experience in healthcare or community setting.
• Candidate must be good at verbal and written communication skills. Ability to communicate using dialects will be an added advantage.
• Able to work on weekends, rotating shifts, and public holidays.
